Referring Doctors
Thank you for referring your patient to Willowbrook Oral and Maxillofacial Surgery. We appreciate the confidence you have in our treatment of your patients.
To expedite treatment for your patient, we recommend you instruct your patient to call our office as soon as possible to schedule their consultation.
Submitting Radiographs
Current radiographs, labelled with the patient's name and the day image was taken, must accompany referrals. If current images are unavailable, this must be indicated on the referral.
Consultation Appointments
The patient pays for consultation appointments at the time of their appointment. Their plan may reimburse them directly if it is a covered expense. Please advise your patient that as a specialist, our fees are higher than the college fee guide and may not be fully covered by their policy.

You may refer your patients to us by:
- Completing Referral Form listed below. You will need to print, complete and email the form along with any relevant X-rays to Reception@WillowbrookOralSurgery.ca or you can fax to (604)532-9687.
Please note that we take the privacy of your patient's data very seriously and take every precaution to protect it. Please ensure all X-rays are correctly labelled with the patient's name and the date they were taken.
